Essential Functions:
· Performs all programs of the health care system within an assigned Clinical Program, functioning with collaborative Implementation and evaluation of clinical Integration Program, is accountable for the delivery of information using evidence based standards, policies, procedures and guidelines of the organization
· Manages the Physician Consortium of Northern Ohio, Ltd (PCNO) Clinical Integration Committee and applicable related functions concerning the Clinical Integration Program and PCNO participating providers
· This position requires the ability to be a team leader with good organizational skills
· Establishes effective working relationships with members of the hospital community, information system personnel, physician committee members, and provider offices
· Must be autonomous and able to lead groups of medical personnel in informational meetings, under direction of Medical Director, PCNO President and other identified members of the multi-disciplinary team
· Serves as an educational resource for internal and external audiences by developing educational material and delivering presentations
· Awareness of and compliance with the Corporate Responsibility Program which includes executing duties in accordance with all policies and regulations affecting one's job; and reporting all concerns or violations of PCNO and/or Mercy Regional Medical Center policies, procedures, and governmental regulations.
Requirements:
· Registered Nurse with 3-5 years of clinical experience
· Case management and quality experience preferred
· Must be pursuing Bachelors Degree of Nursing or higher level of Education within 5 year period
